Utility Billing Account Change Form
This form is used to change the address to which a bill goes and/or add a tenant or property manager to an account.
Direct Pay Authorization Form
This form is used to request that payment for the utility bill be automatically withdrawn from a customer’s checking account. The form is filled out and signed and a voided blank check is attached.
Request for Final Read
This form is used to request a final water bill reading be done for a property that is being sold. The form is generally completed by the title company handling the closing of the property. It is requested that the title company submit the request 72 hours before the date of closing.
